1. Create your account: Please enter your Child’s name and your email in this section. If you mistakenly enter your name, please complete the process and send us an email to correct it.

2. Once your account is created, select Book by service: General Intake.

Select any date and time. Note that this is not a set appointment. It is the allotted time for our Intake Coordinator to review your file. You may be contacted earlier/later by email Intake Coordinator if:

A) We have all the information required and you have been assigned to the waitlist or for a service.

B) We require more information or additional documents.

3. Once you have completed the process, please scroll to Item 2 and complete the Intake Form. This must be completed for us to review your file.
